Video-assisted thoracic surgery pneumonectomy: the first case report in Poland

چکیده
Minimally invasive video-assisted thoracic surgery (VATS) lobectomy has proved to be equal and in some aspects superior to open lobectomy in T1 and T2 lung cancers. Indications for VATS pneumonectomy however are still not clearly defined and strictly limited. The minimally invasive VATS pneumonectomy can be undertaken in patients with centrally located tumors without extended invasion of the large pulmonary vessels, chest wall, pericardium, mediastinal structures or proximal part of the main bronchus and when sleeve resection is not feasible (T2). We present a case of a patient who underwent left VATS pneumonectomy due to left lung cancer. Based on the preoperative examinations any kind of less extensive resection than pneumonectomy was excluded. Our VATS technique consisted of three incisions: two ports and a 5-6 cm long utility incision without any kind of rib spreading. The whole of the procedure was controlled on the monitor via a thoracoscope and the technique of resection was described in detail. Total surgery time was 130 min and blood loss was 150 ml. The chest tube was removed on the 2(nd) post-operative day and the patient was discharged home on the 8(th) postoperative day. The final histopathological examination confirmed squamous cell lung cancer (T2aN0M0 stage IB). In the authors' opinion VATS pneumonectomy should be performed only in centers with extensive experience in minimally invasive VATS lobectomy. Despite limited indications for VATS pneumonectomy, if the patients fulfill the VATS pneumonectomy inclusion criteria they may gain from all the advantages of minimally invasive techniques.

Thoracoscopic Pneumonectomy for Non-Small Cell Lung Cancer (NSCLC): A Case Report and Review of the Literature
Minimally invasive resection of lung cancer remains surprisingly uncommon in comparison to the adoption of similar techniques in other surgical disciplines. The reported use of video-assisted thoracic surgery (VATS) lobectomy for anatomic resections in comparison to open lobectomy ranges from 10% 40% even though it has been demonstrated to be equivalent in safety and accomplishes the same oncol...
متن کاملVideo-assisted contralateral treatment for bronchial stump diastasis after left pneumonectomy.
Postoperative bronchial stump failure is a life-threatening complication, and several surgical approaches and procedures have been developed to close the stump. In this report, we describe a case of left mainstem bronchial stump diastasis after pneumonectomy for lung cancer, in which the bronchial stump was re-closed using a contralateral approach with video-assisted thoracic surgery, with good...
متن کاملSingle-stage bilateral thoracic surgery via a combined VATS and open approach for left central bronchogenic carcinoma with carinal invasion: report of two cases
BACKGROUND Surgery for patients with left central bronchogenic carcinoma invading the carina is challenging due to the complexity of left sleeve pneumonectomy, carinal resection, and airway reconstruction and management. Here we describe a modified approach to overcome this problem. CASE PRESENTATION Between March 2011 and September 2012, two patients with left central bronchogenic carcinoma ...
متن کاملUniportal video-assisted thoracoscopic pneumonectomy.
Video-assisted thoracoscopic surgery (VATS) was introduced nearly 2 decades ago and has experienced an exponential increase for lung cancer treatment. A pneumonectomy can be performed by video-assisted thoracoscopic surgery and the lung usually fits through the incision as usually used for VATS lobectomy. The most common approach for pneumonectomy is undertaken with 3 or 4 incisions, including ...
متن کاملDetermination of the Effects of Digoxin on the Right Ventricular Function in Patients Undergoing Pneumonectomy
Introduction: Pneumonectomy is the standard treatment of lung cancer, even though patients should undergo several evaluations before surgery; deterioration of cardiopulmonary function after pulmonary resection is inevitable. We have evaluated the effects of digoxin on the improvement of right ventricular function and prevention of probable complications after lung resection surgery. Materials a...
